This is the complete CareFlow US Independent Contractor Services Agreement, including the embedded Business Associate Agreement (Section 17). By checking the boxes on the checkout page and clicking "Agree & Start My Campaign," you acknowledge having read and agreed to all 18 sections below.
This Independent Contractor Services Agreement ("Agreement") is entered into between CareFlow US LLC ("CareFlow," "we," or "us") and the client entity identified on the checkout page ("Client" or "you"). The Effective Date is the date the Client checks both acknowledgment boxes and clicks "Agree & Start My Campaign." By doing so, the Client acknowledges having read, understood, and agreed to all terms in this Agreement.
What checking the box means
By checking both boxes on the checkout page and clicking "Agree & Start My Campaign," you are executing a binding electronic signature under the ESIGN Act (15 U.S.C. §7001). This has the same legal force and effect as a handwritten signature on a paper contract.
You acknowledge that you have read, understood, and agreed to all 18 sections of this Agreement, including the 3-campaign minimum, no-refund policy, indemnification terms, binding mediation clause, and the embedded Business Associate Agreement (Section 17). Your IP address, timestamp, and BAA version (v1.0) will be permanently logged as compliance records.
